Registration

Payment in full or a minimum of $750.00 ( non refundable) must accompany the registration. The balance of the program is due 90 days prior to the start of the program. This fee includes tuition, course materials, course syllabus and lunches. Please make check payable to Eubank Teaching Institute.

Cancellation

Due to costs associated with establishing, holding and refilling participant slots, the following cancellation policy has been established. There is a $750 non refundable deposit. Change or cancellation after 90 days will result in a total loss of course tuition. All changes or cancellations must be received in writing. You may email them to Krista@therightfitdentist.com, or fax them to the attention of Krista at 972-867-2219.
